How to Enrol

Eligibility

  1. Practising in a private healthcare facility; and
  2. Being included in the General Register in accordance to the Dentist Registration Ordinance (Cap. 156 of the laws of Hong Kong) and holding a valid practising certificate; and
  3. Enrolled as a healthcare provider (HCP) in the Electronic Health Record Sharing System (eHealth); and
  4. Being listed in the Primary Care Directory (PCD).

Step 1: Submit e-Enrolment form via eHealth

  • Login to eHealth platform
  • Click “Administration”
  • Select “PDCC – Private Dentist Enrolment”

Step 2: Provide the required information via eHealth platform

  • Read and agree to the Terms and Conditions, Undertakings and Declaration, and Personal Information Collection Statement
  • Input necessary information
    1. Personal particulars
    2. Information of clinic
    3. The co-payment amount for the subsidised services
    4. Bank account information
  • Submit the enrolment

Step 3: Submit the following supporting documents

Email the following supporting documents and completed forms to am3_cds@dh.gov.hk after registration within ten (10) calendar days:

  1. Certified true copy of:
    • Valid Practicing Certificate
    • Bank correspondence (e.g. bank statement within 6 months with name of the bank, bank account number and the name of the account holder)
    • Business Registration Certificate
  2. Duly signed and completed:
    • Authority for Payment to a Bank
    • Clinic Administrator Enrolment Form
    • Dental Hygienist Enrolment Form (if applicable)
    • Form for Fee of PDCC Non-subsidised Items
    • Form for Information Displayed on the Website (if applicable)

Announcement of application result

Programme Office will notify the registered dentist the result of his application via email within thirty (30) calendar days of his submission of requisite information and supporting documents.

Provider-based Enrolment

To streamline the enrolment process for clinics in dental groups, eligible registered dentists under their group practice can enrol in the PDCC through their authorised Clinic Administrators by provider-based enrolment through eHealth.
